Эозинофильный эзофагит у детей и подростков: современное состояние проблемы

Цель обзора. Представить актуальные данные относительно эпидемиологии, патогенеза, клинических проявлений и терапевтических подходов к лечению эозинофильного эзофагита (ЭоЭ) с акцентом на детей и подростков.

Основные положения. Современные эпидемиологические данные свидетельствуют о неуклонном росте заболеваемости ЭоЭ в детской популяции во многих странах мира. Ключевую роль в развитии ЭоЭ играют генетические факторы, атопия и гиперчувствительность к пищевым антигенам. Клинические проявления ЭоЭ у детей зависят от возраста и включают нарушения питания, рвоту, абдоминальную боль, дисфагию и застревание пищи в пищеводе. Диагностика ЭоЭ основывается на сочетании характерных клинических симптомов и гистологической картины эозинофильной инфильтрации слизистой пищевода при исключении других причин эозинофилии. Основными направлениями терапии ЭоЭ являются диетотерапия с элиминацией причинно-значимых продуктов, медикаментозное лечение топическими кортикостероидами и эндоскопическое расширение стриктур пищевода.

Заключение. Дальнейшие исследования необходимы для более глубокого понимания молекулярных механизмов ЭоЭ, разработки новых диагностических биомаркеров и оптимизации терапевтических подходов. Ввиду хронического рецидивирующего течения ЭоЭ большое значение имеет долгосрочное динамическое наблюдение пациентов мультидисциплинарной командой специалистов для улучшения качества жизни и предотвращения осложнений.
