Эффективность методов лечения бесплодия у пациенток с эндометриозом I–II стадии

Цель. Оценить эффективность хирургического лечения и методов вспомогательных репродуктивных технологий (ВРТ) при бесплодии, связанном с эндометриозом I–II стадии, по классификации revised American Society for Reproductive Medicine (rASRM).

Дизайн. Рандомизированное исследование.

Материалы и методы. В исследование включены 62 женщины от 25 лет до 41 года с бесплодием и эндометриозом I–II cтадии, по rASRM, установленным на основании клинических симптомов и данных магнитно-резонансной томографии. Пациентки рандомизированы на две группы: группа 1 (лапароскопия) — хирургическое лечение бесплодия, связанного с эндометриозом, с последующим планированием самостоятельной беременности в течение 12 месяцев (n = 35); группа 2 (ВРТ) — лечение бесплодия, связанного с эндометриозом, с использованием методов ВРТ (n = 27). Репродуктивные исходы в группе 1 оценивались по истечении 1 года с момента оперативного лечения, в группе 2 — по результатам трех попыток переноса эмбрионов в рамках применения методов ВРТ.

Результаты. Частота наступления беременности (группа 1 — 34,3%; группа 2 — 33,3%; p = 0,937), прерывания беременности (группа 1 — 25%; группа 2 — 44,4%; p = 0,397), родов (группа 1 — 20%; группа 2 — 14,8%; p = 0,643) статистически значимо не различалась в группах лечения. Ни один из изученных клинико-анамнестических параметров не влияли на вероятность наступления беременности.

Заключение. Хирургическое лечение и использование методов ВРТ при лечении бесплодия у пациенток с эндометриозом I–II стадии имеют равную эффективность. Выбор метода может зависеть от предпочтения пациентки, данных анамнеза, особенностей и стадии заболевания, наличия сопутствующей патологии, а также других факторов бесплодия.
