Оценить распространенность метаболического синдрома (МС) у пациентов с псориатическим артритом (ПсА) и определить связи сывороточных ангиопоэтин-подобных белков (АППБ) различных типов с характером метаболических нарушений при данном заболевании.</p></sec><sec><title>Дизайн</title><p>Дизайн. Одноцентровое поперечное клиническое исследование.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследованы 45 пациентов старше 18 лет с ПсА (25 женщин и 20 мужчин; средний возраст — 47,5 ± 12,2 года; длительность заболевания — 7,9 ± 6,2 года), преимущественно с высокой (индекс DAS (Disease Activity Score) &gt; 3,7) активностью заболевания (67%). Наличие псориаза подтверждено в 100% случаев, ожирение различной степени выраженности диагностировано у 17 человек. Стандартное клинико-лабораторное обследование всех участников было дополнено определением сывороточных АППБ 3, 4 и 6 типов методом иммуноферментного анализа.</p></sec><sec><title>Результаты</title><p>Результаты. Однофакторный дисперсионный анализ не выявил связи между активностью ПсА и уровнями АППБ. МС согласно критериям Международной диабетической федерации (IDF, 2006) диагностирован у 27 (60%) пациентов с ПсА. Межгрупповое сравнение показателей АППБ различных типов продемонстрировало увеличение уровня АППБ 6 типа при наличии МС (р = 0,018). Артериальная гипертензия ассоциировалась с высокими уровнями АППБ 6 типа у пациентов с ПсА при наличии МС. Содержание АППБ 3 и 6 типов у пациентов с ожирением было существенно выше, чем у лиц с избыточной массой тела (р = 0,002) и у больных ПсА без ожирения (р = 0,024).</p></sec><sec><title>Заключение</title><p>Заключение. Концентрации сывороточных АППБ 3, 4 и 6 типов изменяются в соответствии с различными метаболическими фенотипами ПсА. АППБ 3 и 6 типов могут использоваться в качестве диагностических маркеров метаболических нарушений у больных ПсА.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To assess the prevalence of metabolic syndrome (MS) in patients with psoriatic arthritis (PsA) and to determine the associations of serum angiopoietin-like proteins (Angptl) of different types with the characteristics of metabolic disorders in this disease.</p></sec><sec><title>Design</title><p>Design. Single-center cross-sectional clinical study.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Forty-five patients older than 18 years with PsA (25 women and 20 men; mean age 47.5 ± 12.2 years; disease duration 7.9 ± 6.2 years) with predominantly high (DAS (Disease Activity Score) &gt; 3.7) disease activity (67%) were examined. The presence of psoriasis was confirmed in 100% of cases, obesity of various severity was diagnosed in 17 people. Standard clinical and laboratory examination of all participants was supplemented by determination of serum Angptl of types 3, 4 and 6 by enzyme-linked immunosorbent assay.</p></sec><sec><title>Results</title><p>Results. One-factor analysis of variance revealed no association between PsA activity and Angptl levels. MS according to the criteria of the International Diabetes Federation (IDF, 2006) was diagnosed in 27 (60%) patients with PsA. Intergroup comparison of Angptl indices of different types demonstrated an increased level of Angptl type 6 in the presence of MS (p = 0.018). Arterial hypertension was associated with high levels of type 6 Angptl in patients with PsA in the presence of MS. The content of Angptl types 3 and 6 was significantly higher in obese patients than in overweight patients (p = 0.002) and in PsA patients without obesity (p = 0.024).</p></sec><sec><title>Conclusion</title><p>Conclusion. Concentrations of serum Angptl types 3, 4 and 6 vary according to different metabolic phenotypes of PsA.
