Оценка эффективности периоперационного управления гликемией у больных ишемической болезнью сердца (ИБС) и сахарным диабетом (СД) 2 типа при использовании систем непрерывных мониторирования уровня глюкозы (НМГ) и подкожной инфузии инсулина (НПИИ) для снижения риска послеоперационных осложнений коронарного шунтирования. Дизайн. Проспективное сравнительное исследование.Материалы и методы. В исследование включены 105 больных со стабильной ИБС и многососудистым поражением коронарного русла, имевших показания к коронарному шунтированию, из которых 52 страдали СД 2 типа. У всех пациентов проводился контроль периоперационной гликемии с помощью системы НМГ в течение трех суток. Пациенты с ИБС и СД 2 типа были разделены на две подгруппы: в первой осуществлялась НПИИ с учетом данных НМГ, во второй — болюсное внутривенное введение инсулина, согласно внутрибольничному протоколу. Для изучения провоспалительного статуса и адипокинового профиля брали образцы периферической венозной крови до операции, через час, 12 ч, на 7-е сутки и через год после коронарного шунтирования.Результаты. Использование НПИИ с учетом данных НМГ более эффективно для управления периоперационной гликемией (через 12 часов после операции в большинстве случаев были достигнуты допустимые значения гликемии (4–10 ммоль/л): 62,5% vs 21,4% во второй подгруппе, р = 0,003). При НПИИ уменьшалась активность системной провоспалительной реакции, что обусловлено снижением уровней высокочувствительного С-реактивного белка через 12 часов и RANTES на 7-е сутки после операции (р = 0,0001, р = 0,035 соответственно для отличий от второй подгруппы). У пациентов с НПИИ уровень лептина на 7-е сутки был значимо ниже, чем в подгруппе болюсного внутривенного введения инсулина (р = 0,039). Наблюдалась отрицательная взаимосвязь между концентрацией лептина на 7-е сутки и использованием НПИИ (r = –0,73 р = 0,025). Через год после операции уровень адипонектина оказался значимо выше у пациентов с НПИИ (р = 0,028). У больных с НПИИ частота развития посткардиотомного синдрома в раннем послеоперационном периоде и длительность госпитализации также были меньше (р = 0,03, р = 0,007 соответственно), однако значимая разница в частоте осложнений в отдаленном периоде не отмечена.Заключение. Использование НПИИ с учетом данных НМГ позволяет эффективно и безопасно управлять нарушенным гомеостазом глюкозы в периоперационном периоде коронарного шунтирования, а также снизить частоту возникновения посткардиотомного синдрома. Ключевые слова: ишемическая болезнь сердца, сахарный диабет 2 типа, коронарное шунтирование, непрерывная подкожная инфузия инсулина.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To assess an efficacy of continuous glucose monitoring system (CGMS) combined with insulin pump therapy for perioperative glycemic control after coronary artery bypass grafting (CABG).Design. Prospective comparative study.Materials and methods. We used CGMS for 72 hours in 105 patients with coronary artery disease including 52 ones with diabetes mellitus type 2 (DM2). Patients with DM2 were divided into 2 subgroups: insulin pump therapy (subgroup 1) and bolus intravenous insulin according to in-hospital protocol (subgroup 2). We analyzed pro-inflammatory markers, adipokines and incidence of postoperative complications in one hour, 12 hours, 7 days and 1 year after CABG.Results. Insulin pump therapy ensured target glycemia in most cases (62.5%) while bolus intravenous insulin injection was followed by normal glucose level in 21.4% of patients (p = 0.003). Insulin pump therapy significantly decreased highly sensitive C-reactive protein after 12 hours (p = 0.0001), RANTES after 7 days (p = 0.035). In patients on insulin pump therapy the leptin level after 7 days was significantly lower than in the subgroup 2 (p = 0.039). There was a negative relationship between the leptin level after 7 days and the use of CGMS combined with insulin pump therapy (r = –0.73; p = 0.025). In patients on insulin pump therapy the adiponectin level after 1 year was significantly higher than in the subgroup 2 (p = 0.028). Insulin pump therapy decreased the incidence of postpericardiotomy syndrome (p = 0.03) and the period of hospitalization (p = 0.007), but there was no significant difference in the rate of complications in the long-term period.Conclusion.
