Изучить роль полиморфизма генов липидного обмена и изменения липидтранспортной системы крови у пациенток с умеренной и тяжелой преэклампсией (ПЭ).</p></sec><sec><title>Дизайн</title><p>Дизайн. Проспективное когортное сравнительное исследование.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проанализированы результаты клинико-анамнестического и лабораторного обследования 130 пациенток с высоким риском формирования ПЭ по данным комплекса пренатальной диагностики первого триместра. Пациентки были разделены на три группы: с реализованной тяжелой ПЭ (I группа, n = 53, в сроке беременности от 23 до 38 недель), с реализованной умеренной ПЭ (II группа, n = 45, в сроке беременности 28–40 недель) и без ПЭ (группа сравнения, или III группа, n = 32, в сроке беременности 28–40 недель). В рамках клинического обследования проведен сбор соматического и акушерско-гинекологического анамнеза, исследовано течение текущей беременности. Биохимический анализ крови осуществлялся при поступлении пациенток в стационар и включал изучение показателей липидного спектра (холестерин, триглицериды, липопротеины высокой, низкой и очень низкой плотности), общего билирубина, мочевины, креатинина, аполипопротеинов (АроА, АроВ). Генотипирование образцов проведено методом полимеразной цепной реакции в режиме реального времени с использованием комплектов реагентов и протокола для определения генетических полиморфизмов. Результаты. По полученным данным, наиболее информативными показателями дислипидемии у беременных, ассоциированными с риском развития ПЭ, являются повышенный уровень липопротеинов очень низкой плотности, коэффициент атерогенности более 3,5 и индекс ApoB/ApoA более 0,9.</p><p>Наличие аллеля T полиморфного локуса ApoE rs429358 в гомо- или гетерозиготном состоянии в генотипе увеличивает шанс формирования ПЭ, а генотип СС полиморфизма ApoE rs429358, наоборот, обладает протективным эффектом в отношении развития этой патологии. Протективный эффект имеет также генотип АА полиморфизма rs708272 гена CETP (G&gt;A), а носительство в генотипе хотя бы одного вариантного аллеля G этого локуса, наоборот, увеличивает риск формирования ПЭ.</p></sec><sec><title>Заключение</title><p>Заключение. Исследование приводит к выводу о значимости показателей липидтранспортной системы, а именно соотношений аполипопротеинов и липопротеинов различной плотности, и полиморфизмов ApoE T&gt;C, CETP G&gt;A в патогенезе и диагностике ПЭ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the role of lipid metabolism gene polymorphism and changes in the lipid-transporting lipid system in patients with moderate to severe preeclampsia (PE).</p></sec><sec><title>Design</title><p>Design. Prospective cohort comparative study.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We analysed results of clinical, anamnestic and laboratory examination of 130 patients with a high risk of PE, following a prenatal diagnostics performed during the first trimester. The patients were divided into three groups: with actual severe PE (group I, n = 53, weeks 23 to 38 of pregnancy); with actual moderate PE (group II, n = 45, weeks 28 to 40 of pregnancy), and without PE (control group, or group III, n = 32, weeks 28 to 40 of pregnancy).</p><p>During the clinical examination, somatic and obstetric-gynaecological history was taken, and the present pregnancy was reviewed. A sample for blood biochemistry was drawn upon hospitalisation; the analysis included lipid parameters (cholesterol, triglycerides, high, low and very low-density lipoproteins), total bilirubin, urea, creatinine, apolipoproteins (АроА, АроВ). Sample genotyping was performed using the realtime polymerase chain reaction with a reagent kit and protocol for genetic polymorphisms.</p></sec><sec><title>Results</title><p>Results. The results show that the most informative parameters of dyslipidaemia in pregnant women, which are associated with the risk of PE, are high levels of very low-density lipoproteins, atherogenic index of over 3.5 and ApoB/ApoA index of over 0.9.</p><p>The presence of allele T of polymorphic locus ApoE rs429358 in homo- or heterozygous state in the genotype increases the risk of PE, while genotype СС of polymorphism ApoE rs429358 has a protective effect against this pathology. Genotype AA of polymorphism rs708272 of gene CETP (G&gt;A) has also a protective effect, while the presence in the genotype of at least one variant of allele G of this locus increases the risk of PE.</p></sec><sec><title>Conclusion</title><p>Conclusion.
