Приведен аналитический обзор современной парадигмы ГСД и ПЭ, их осложнений, а также перинатальных и отдаленных исходов для матери и ребенка. Систематизированы данные о клинико-анамнестических и лабораторных факторах риска ПЭ при ГСД. Описана тесная взаимосвязь механизмов развития ПЭ и ГСД — ангиогенного дисбаланса и метаболических нарушений в плаценте.</p></sec><sec><title>Заключение</title><p>Заключение. ГСД признан независимым фактором риска развития ПЭ, причем этот риск тем больше, чем менее ГСД компенсирован. Синдром поликистозных яичников и повышенный индекс массы тела в качестве сопутствующих факторов нарушения метаболизма также увеличивают риск развития ПЭ. Предполагаемая патогенетическая общность ГСД и ПЭ заключается в наличии окислительного стресса и метавоспаления, усиливающих инсулинорезистентность и вызывающих эндотелиальную дисфункцию.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective of the Review</title><p>Objective of the Review: To summarize the proven prognostic and diagnostic criteria for gestational complications caused by a combination of gestational diabetes mellitus (GDM) and preeclampsia (PE).</p></sec><sec><title>Key points</title><p>Key points. There is an analytical review of the modern paradigm of GDM and PE, their complications, as well as perinatal and long-term outcomes for mother and child. The data on clinical, anamnestic and laboratory risk factors of PE in GDM are systematized. A close relationship between the mechanisms of PE and GDM development — angiogenic imbalance and metabolic disorders in the placenta is described.</p></sec><sec><title>Conclusion</title><p>Conclusion. GDM is recognized as an independent risk factor for the development of PE, and this risk is the greater the less GDM is compensated. Polycystic ovary syndrome and an increased body mass index as concomitant factors of metabolic disorders also increase the risk of developing PE.
