Изучить показатели минерального обмена и минеральной плотности костной ткани у пациентов с длительным приемом антиконвульсантов и здоровых добровольцев.</p></sec><sec><title>Дизайн</title><p>Дизайн. Наблюдательное кросс-секционное исследование с включением двух групп сравнения.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследованы 100 пациентов, получавшие антиконвульсанты более 12 месяцев, и 58 здоровых добровольцев, не принимавшие противоэпилептические препараты. Всем участникам исследования проведены клиническое психоневрологическое обследование, анализ лабораторных показателей микроэлементов и гормонов, участвующих в метаболизме костной ткани, и радиологическое обследование минеральной плотности костной ткани c использованием количественной компьютерной томографии (КТ-денситометрии).</p></sec><sec><title>Результаты</title><p>Результаты. Сравнительный анализ лабораторных параметров микроэлементов и гормонов, связанных с костным обменом, выявил статистически значимые различия средних показателей кальция ионизированного, витамина D, тироксина свободного и пролактина между группами пациентов, получавших антиконвульсанты, и здоровых участников (p (U) = 0,044, p (U) = 0,052, p (U) = 0,001, p (U) = 0,003 соответственно). У больных с длительным приемом антиконвульсантов различных поколений, в отличие от здоровых лиц, установлены статистически значимо более высокие уровни дефицита витамина D (p (χ2) = 0,010, p (χ2) = 0,016). При сравнительном анализе показателей витамина D и изменений минеральной плотности костной ткани по данным КТ-денситометрии в группах больных с длительным приемом антиконвульсантов и здоровых участников не удалось выявить статистически значимых различий (p (χ2) = 0,851, p (χ2) = 0,174 соответственно). У пациентов, принимавших антиконвульсанты, по сравнению со здоровыми лицами уровень ионизированного кальция был существенно выше (p (χ2) &lt; 0,001), что является маркером преобладания процесса остеорезорбции над остеогенезом.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты исследования подтверждают отрицательное влияние антиконвульсантов на минеральной обмен и плотность костной ткани. Полученные данные могут иметь важное значение для разработки диагностических и лечебных мероприятий у пациентов с длительным приемом антиконвульсантов с целью своевременной профилактики нарушения метаболизма костной ткани и развития остеопороза.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To investigate parameters of mineral metabolism and bone mineral density in patients with long-term administration of anticonvulsants and healthy volunteers.</p></sec><sec><title>Design</title><p>Design. Observational cross-sectional study with including two comparison groups.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 100 patients receiving anticonvulsants for more than 12 months and 58 healthy volunteers without antiepileptic drugs. Clinical neuropsychiatric examination, analysis of laboratory parameters of microelements and hormones associated with bone metabolism, and radiologic examination of bone mineral density using quantitative computed tomography (CT-densitometry) were performed on all study participants.</p></sec><sec><title>Results</title><p>Results. The comparative analysis of laboratory parameters of micro-elements and hormones associated with bone metabolism revealed statistically significant differences in mean values of calcium ionized, vitamin D, free thyroxine and prolactin between the groups of patients receiving anticonvulsants and healthy participants (p (U) = 0.044, p (U) = 0.052, p (U) = 0.001, p (U) = 0.003, respectively). Statistically significantly higher levels of vitamin D deficiency (p (χ2) = 0.010, p (χ2) = 0.016) were found in patients with long-term use of anticonvulsants of different generations, in contrast to healthy individuals. The comparative analysis of vitamin D indices and changes in bone mineral density according to CT-densitometry in the groups of patients with long-term anticonvulsants and healthy participants showed no statistically significant differences (p (χ2) = 0.851, p (χ2) = 0.174, respectively). Patients receiving anticonvulsants had significantly higher levels of ionized calcium (p (χ2) &lt; 0.001) compared to healthy participants, which is a marker of the prevalence of the process of osteoresorption over osteogenesis.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results of the study confirm the negative effect of anticonvulsants on mineral metabolism and bone density.
